RN2 Immunizations

DOCUMENTATION of IMMUNIZATIONS:

• Hepatitis B – 3 doses
• DTaP/DTP/DT/Td – at least 4 doses, and 1Tdap booster (if at least 5 years have elapsed since last dose, Tdap booster is usually due by 7th grade)
• Polio – at least 3 doses
• Measles – 2 doses
• Mumps – 2 doses
• Rubella – 2 doses
• Varicella – 2 doses or written proof of having had chicken pox provided by physician

The flu vaccine is no longer required. The Massachusetts Department of Public Health is removing the flu vaccination requirement for attendance in schools during the 2020-2021 school year. If you have received a letter from the FCTS Health office requesting verification, please disregard.

All students entering Grades 7 and 11 need meningococcal conjugate vaccine, MenACWY (brand names Menveo or Menactra) for school entry. Grade 7 entry: 1 dose of MenACWY for all students. Grade 11 entry: 1 booster dose of MenACWY received on or after 16 years of age. (1 or more doses of MenACWY are acceptable as long as 1 dose was received on or after 16 years of age. The above guidelines are minimum legal requirements for students to attend school in Massachusetts. Exclusion from school is necessary for students not meeting MINIMUM REQUIREMENTS.
